The standard approach for treating acne is
multi-drug therapy.
The combination of erythromycin and benzoyl peroxide
has proven to be well tolerated and effective
topical treatment for acne vulgaris.
Multicenter randomised study in 223 patients with
moderate to moderately severe acne who received
erythromycin /5% benzoyl peroxide or matching
vehicle control for 8 weeks.
Investigators found the combination was more
effective for pustular and papular lesions than
any of the other treatments

| Pharmacology: |

Erythromycin is a bacteriostatic macrolide antibiotic,
but it is a bactericidal in high concentration.
Its effect bases on the inhibition of bacterial
protein synthesis.
Although the mechanism by which topical erythromycin
acts in reducing inflammatory lesions of acne
vulgaris is unknown, it is may be due to its antibiotic
action (it works against most G +ve & G ve
microorganisms)

The topical treatment with erythromycin as alternative
to systemic treatment
Clinical trials the authors Fulton & Pablo
(1974) report :- an improvement of acne after
topical application of erythromycin as well as
a reduced concentration of free fatty acids in
the skin surface lipids.
Moreover aknemycin oint emulsified the sebum
in the acne & remove it from the duct of the
sebaceous gland.

| Absorption |

After using Akne-mycin 2% sol. to large
areas of forehead, back, shoulder and upper
arm.
Blood samples have been taken before 2 &
4 weeks after. No erythromycin was detected
in the serum 0.3 ug/ml. |

Aknemycin 2% has significant reduction effect
in the propionibacterium acnes as well as
the number of total germs. |
